Admission Process
Our admission process is broken down into three phases: Inquire, Initiate and Implement. During the no obligation Inquire phase, information is gathered about your child, your concerns, and qualifications for services. This provides us with the information we need to let you know if our services are a good fit for your child and family and if our services will be covered by your insurance.
The Initiate phase is for families who want to move forward with pursuing services. At this time, we will conduct a comprehensive assessment to evaluate the specific strengths and needs of your child as well as have you fill out more detailed information about your child’s history, your concerns and your expectations from services. This will guide us in creating an Individualized Treatment Plan for your child.
The final phase is to Implement the treatment plan and start services! See below for a breakdown of the admission process phases and requirements.
Inquire
Online Admission Application
Submitting your information securely using the online admission application is the quickest and most effective way of acquiring information on services, benefits and beginning therapy.
Provide Documents
Please submit documents by email, fax or through our secure Online Application so that we can begin checking coverage with your insurance provider. Required documents include: Diagnostic Report and Insurance Card
Intake
Our administrative team will verify your ABA coverage and contact you to let you know about treatment options, anticipated cost of services and provide you with information on moving forward with treatment
Initiate
Assessment
Our team will work with you to schedule your child's initial evaluation. A comprehensive intake packet will need to be completed and returned prior to your assessment time.
Discuss a Plan
You will have the opportunity to meet with a Board Certified Behavior Analyst (Abree or Sarah) following assessment to collaborate on a treatment plan and services schedule.
Implement
Start Services
We will contact you to let you know when your child’s services have been authorized and set a start date. You will be instructed to complete the Welcome Packet which contains important information pertaining to safety, rights and responsibilities while receiving therapy.
